Plan the way off before you price a 300 mg Neurontin bottle. The labelled start for postherpetic neuralgia is one 300 mg capsule on day one, two on day two, three on day three, then a climb toward 1800 mg a day if the pain needs it. Saturable gut transport means each extra milligram buys a smaller share. Kidneys, not the liver, clear almost all of it. Ordinary mechanical backache is not a labelled job, and the published trials in that picture have been thin. Opioids and alcohol stack the sedation and the breathing risk named in 2019. Three hundred milligrams is a start, not a month

Plan the way off before you ask what ninety capsules cost. A 300 mg Neurontin capsule is the labelled day-one piece for adult postherpetic neuralgia, then 300 mg twice on day two, then 300 mg three times on day three. The climb can continue to 1800 mg a day (600 mg three times daily) if the pain still needs it. Clinical studies used 1800 to 3600 mg a day and did not show extra benefit above 1800 mg.

Dropping the capsule because a month felt expensive, or because a weekend felt clear, is the error this profile exists to stop. If the dose is reduced, stopped, or swapped, the label wants a gradual change over at least one week, longer if the prescriber says so. Abrupt stop can raise seizure frequency in people with epilepsy and has been followed by anxiety, insomnia, nausea, pain, and sweating in post-marketing reports.

Saturable absorption is the other reason a 'stronger bottle' is a poor plan. Bioavailability is about 60 percent at 900 mg a day, 47 percent at 1200, 34 percent at 2400, 33 percent at 3600, and 27 percent at 4800 mg a day, all in three divided doses. The gut transporter fills up. Doubling the milligrams does not double the blood level.

Stopping: one week is the floor

Minimum one week is the labelled floor for a reduction, a stop, or a swap. Longer is allowed and often wiser after months at 1800 mg or more. There is no single published milligram-per-week cartoon on the Neurontin label. The clinic writes the steps. A webpage that sells a 'four-day detox' is not a taper.

Seizure risk is the hard reason in people who take it as an antiepileptic. Withdrawal symptoms in others have included anxiety, insomnia, nausea, pain, and sweating. Restarting the capsule has settled some of those pictures within a few days, after which a slower walk down can resume. I do not write a 300 mg price until the off-ramp is on paper.

Overdose reports include double vision, tremor, slurred speech, drowsiness, altered mentation, dizziness, lethargy, and diarrhea. Fatal respiratory depression has been reported with gabapentin overdose alone and with other CNS depressants. That is emergency care, not a consult thread.

Day one is one 300 mg capsule. Day two is 300 mg twice. Day three is 300 mg three times. After that the prescriber can step toward 1800 mg a day. Judging the drug on day four is how people quit a titration that never reached a trial dose. Nerve-pain benefit, when it comes, often needs weeks at a stable maintenance dose, not a weekend sample.

Split the day. A 5-to-7-hour half-life does not cover a once-daily swallow. The seizure label is explicit: do not let more than 12 hours pass between doses on a three-times-daily schedule. Missing the midday capsule and doubling at night is how ataxia and sleepiness stack.

Elderly kidneys write a lower ceiling even when the birthday is the only 'disease.' In the postherpetic trials, 30 percent of treated adults were 65 to 74 and 50 percent were 75 or older. The larger effect in the oldest group may simply be more drug left in the blood because clearance fell. Peripheral edema and ataxia tended to rise with age. Start low. Climb slower than the three-day cartoon if the eGFR is already sliding.

Ordinary backache is not on that card

Mechanical low back pain and garden sciatica are the scripts that fill a lot of 300 mg bottles and then disappoint. Systematic reviews of gabapentinoids in ordinary chronic backache and radicular pain have found benefit close to placebo and a real rate of dizziness, sleepiness, and unsteadiness. Several national back-pain pathways now say that out loud and point clinicians elsewhere first. A neighbor who 'uses it for the back' is not a labelled indication, and a cheap ninety-count does not change that math.Diabetic nerve pain, fibromyalgia, generalized anxiety, alcohol withdrawal, hot flashes, and restless legs are also off-label. Some of those pictures have better data than others. None of them change the absorption math or the taper rule. A 2004 Department of Justice settlement with Warner-Lambert documented promotion for unapproved uses, including various pain states and bipolar disorder, ahead of the trial record. That history is why a 300 mg price page that never mentions postherpetic neuralgia or seizures should make you suspicious.

Pregabalin carries labelled jobs gabapentin does not, fibromyalgia and diabetic neuropathy among them, because its absorption is predictable. Cost still pushes many clinics toward generic gabapentin anyway. Cost is not evidence. If the pain is a sprained lumbar disc without a neuropathic signature, I would rather you not buy ninety capsules because a neighbor 'uses it for the back.'

Saturable gut: why more milligrams buy less

L-type amino acid transport in the small bowel carries gabapentin. The carrier saturates. That is why the percent absorbed falls as the daily total climbs, and why a person who 'feels nothing at 900' will not automatically feel twice as much at 1800. Pregabalin uses a less saturable path and keeps a high bioavailability across its range. That PK difference, not marketing, is why the two gabapentinoids are not copy-paste switches.

Food nudges absorption only slightly: about a 14 percent rise in AUC and Cmax. Take the capsule with or without a meal and stay consistent. An aluminum-magnesium antacid (the old Maalox studies) cuts mean bioavailability by about 20 percent. The label wants gabapentin at least 2 hours after that antacid. Less than 3 percent of circulating drug is bound to plasma protein. Apparent volume of distribution after 150 mg intravenous was 58 plus or minus 6 L.

Elimination half-life is 5 to 7 hours and does not change with dose or with repeated dosing. Plasma clearance and renal clearance track creatinine clearance. Almost no hepatic metabolism. Hemodialysis removes the drug, which is why the renal table includes a post-dialysis supplement. In people with epilepsy, steady-state CSF levels sit at about 20 percent of the matching plasma level.

A GABA name that never met a GABA receptor

Chemists at Parke-Davis built a GABA look-alike in the 1970s hoping it would walk into the brain and act like the inhibitory transmitter. It does cross into the brain. It does not bind GABA-A or GABA-B receptors, does not block GABA reuptake, and does not change how the body makes or breaks GABA. The name still causes bad prescribing.

Alpha-2-delta-1 on voltage-gated calcium channels is the actual binding site. Less calcium into a nerve terminal means less glutamate and substance P onto the next cell. Damaged nerves, the kind left after shingles, tend to show more of that subunit, which is one reason postherpetic neuralgia responds and a fresh sprained back often does not.

Anxiety and insomnia scripts written on the GABA story alone sit on the thinnest evidence. The two FDA jobs are postherpetic neuralgia in adults and adjunctive treatment of partial-onset seizures, with or without secondary generalization, in people three years and older. Everything else is off-label, including the ordinary backache that still fills a lot of 300 mg bottles.

Postherpetic neuralgia is the burning, lingering pain after a shingles rash. The adult start is the 300 mg day-one, day-two, day-three climb already named. Effective study doses ran 1800 to 3600 mg a day. Extra benefit above 1800 mg was not shown. In those trials, 16 percent of 336 people on gabapentin stopped for an adverse reaction versus 9 percent of 227 on placebo. Dizziness, somnolence, and nausea led the withdrawals.

Somnolence hit 21 percent on drug versus 5 percent on placebo in the postherpetic trials. Dizziness hit 28 percent versus 8 percent. Those two numbers explain most of the 'I cannot drive yet' letters. The label says not to drive or use heavy machines until the person knows how the capsule treats them.

Partial-onset seizures in people 12 and older start at 300 mg three times a day. The usual maintenance band is 300 to 600 mg three times a day. Long-term studies have used up to 2400 mg a day. A few people have taken 3600 mg a day for a short stretch. The longest gap between the three daily doses must not exceed 12 hours. In the controlled epilepsy trials above age 12, somnolence was 19 percent versus 9 percent, dizziness 17 versus 7, ataxia 13 versus 6.

Children 3 to 11 years have a weight-based climb over about three days. Ages 3 to 4: 40 mg/kg/day in three doses. Ages 5 to 11: 25 to 35 mg/kg/day. Starting range is 10 to 15 mg/kg/day. Postherpetic neuralgia is not an approved pediatric job.

Opioids, alcohol, and the 2019 breathing warning

Respiratory depression, including fatal cases, has been reported when gabapentin sits with opioids or other CNS depressants, or when the lungs are already weak. Morphine, hydrocodone, oxycodone, and buprenorphine are named. The 2019 FDA warning told clinicians to start low and to watch breathing and sedation when those combinations cannot be avoided. Alcohol is the household version of the same stack.

Hydrocodone exposure falls when gabapentin is added. Morphine plus gabapentin needs a watch for sleepiness and slow breathing. None of that is a reason to stop an opioid from a webpage. It is a reason to tell the prescriber both names before the 300 mg climb starts.

Misuse and abuse have been reported after approval, mostly in people who already used other substances, sometimes at higher than labelled doses for unapproved jobs. Physical dependence has rare post-marketing reports after abrupt stop of high, off-label doses: agitation, disorientation, confusion that settled when the drug was restarted. The abuse and dependence potential were not measured in dedicated human studies. Still, a cheap 300 mg bottle is not harmless inventory.

Close the taper before you price the bottle

Write the indication first. Postherpetic neuralgia and add-on partial seizures are the two labelled jobs. Ordinary backache is a weak bet. Write the kidney number second. Write the climb third: 300, then 600, then 900, then a pause long enough to see whether the nerve pain moved. Write the off-ramp last, at least a week, before anyone talks about a cash board.

Name every opioid, sleep drug, and the evening wine. The 2019 breathing warning is about that stack, not about a single 300 mg capsule on an empty list. Day-to-day titration notes live on the nerve-pain dispatch. The physician who answers the thread is Dr. Elin Dahl.

Ordinary mechanical backache after a lift is not a labelled Neurontin job. The two FDA indications are postherpetic neuralgia in adults and add-on treatment of partial-onset seizures from age three. Reviews of gabapentinoids in ordinary chronic back pain and sciatica have found little more than placebo and a real rate of dizziness and sleepiness. Several national pathways already tell clinicians to look elsewhere first.

A jump to 300 mg three times daily on day one is also faster than the postherpetic climb (300, then 600, then 900). If the pain is truly neuropathic and the clinic still wants this drug, I would rather see a slower start and a written taper than a ninety-count bought because the neighbor uses it 'for the back.' Ask what finding made this a nerve-pain drug rather than a muscle-and-disc drug. Price comes after that answer.

Do not drop it this weekend. If gabapentin is reduced, discontinued, or replaced, the label wants a gradual change over at least one week, and longer if your clinician says so. People with seizures can have more seizures after an abrupt stop. Other people have reported anxiety, insomnia, nausea, pain, and sweating after a sudden halt.

Two months at 900 mg a day is enough exposure that I would not use a three-day home taper either. Call the prescriber, ask for a written step-down, and keep enough capsules to walk that plan. Feeling better is a reason to plan a stop, not a reason to empty the drawer on Friday. The price of ninety never includes permission to stop cold.

My eGFR is 38. The script is still 300 mg three times a day. Is that the labelled clock?

Creatinine clearance between 30 and 59 mL/min sits on a 400 to 1400 mg total daily band, usually given twice a day, not three times at 300 mg (900 mg) as if the kidneys were normal. The label table for people 12 and older is keyed to clearance, not to a birthday. Gabapentin is cleared unchanged in the urine. The half-life stretches as clearance falls.

I would take that script back to the clinic with the latest eGFR before I paid for ninety. A lower total, split twice daily, is the usual correction. Hemodialysis would add a post-dialysis supplement (125 to 350 mg after the run). Do not invent the new milligrams at the counter. Do not ignore the number because 300 mg 'is a small capsule.'

The 2019 FDA warning covers serious, including fatal, breathing problems when gabapentinoids sit with opioids or when the lungs are already weak. Hydrocodone is named. Alcohol is the same kind of stack even though it is not a tablet.

Hydrocodone levels can fall when gabapentin is added, which is a separate PK point from the sedation. Morphine plus gabapentin needs a watch for sleepiness and slow breathing. I start low in that mix and I ask someone in the household to notice if night breathing changes. This is not a reason to stop hydrocodone from a gallery page. Saturable absorption is the first answer. About 60 percent of a 900 mg daily total is absorbed. At 1800 mg a day you are not absorbing twice that fraction; the labelled figures fall toward the mid-30s as the daily total climbs. The transporter fills. More milligrams buy less extra drug.

Wobbliness is on-label. Ataxia was 13 percent versus 6 percent in the adult epilepsy trials. Dizziness was 28 percent versus 8 percent in the postherpetic trials. Extra benefit above 1800 mg a day was not shown in the pain studies, even though some people were taken to 3600 mg. Self-raising past the written dose is also how misuse reports start. Go back to the prescriber with a pain diary instead of another bottle.

A forum schedule cannot see your creatinine, your opioid list, or whether the pain is postherpetic or a disc. The labelled adult pain climb is 300 mg on day one, 600 mg on day two, 900 mg on day three, then a clinician-guided rise toward 1800 mg a day. The labelled stop is at least one week, not a forum 'jump off.' Seizures, DRESS, anaphylaxis, and the 2019 breathing warning are clinic problems.
